most likely hardware but you'll need to go through possible software fixes, before you can outright declare it's a hardware problem
SW: clear camera app data and cache, factory reset and possibly flash back stock firmware
HW: clean the glass covering the lens with clean dry cloth, use your fingers to gently tap on the camera area
