I understand that i must flash twrp image on boot and recovery partition to boot my watch to wtrp and make some changes and then i must flash boot image to boot my watch to android?
When i only flash recovery partition could i find twrps file and change tw_rotation to 180?
After that i can use adb reboot recovery command to go to twrp?

you will need to clarify from the OP of the guide you're following as to why you need to flash to both boot and recovery, normally you should flash twrp to recovery partition only
answer to your 3 questions below
the changes you speak about, based on the instructions is same as flashing boot.img back to boot partition

(16-10-2019, 10:59 AM)ktl20 I have used Hovatek Twrp porter and it works. I have twrp recovery on my watch and everything is fine except that is very small font size and on this watch its hard to do something with twrp. Is there any way to change resolution or font size in twrp?

try this
unpack both the twrp from our tool and your previous twrp using the guide @ https://www.hovatek.com/forum/thread-15817.html
copy and replace the items from /ramdisk/twres in your previous twrp to that from our tool
repack then flash and check if the fonts sizes are okay
