ensure the phone is in fastboot mode and drivers have been properly installed on the PC

BTW why don't you do a simple factory reset in recovery mode?

Download kingo root on your pc, install the drivers (you will need Internet connection). You don't need to root. Just install drivers. Your phone should connect to adb.
