What's the difference between system and system image when taking a twrp backup. More specifically on infinix hot 4 x557 A1

They're the same thing as they both refer to the same partition. "System image" is just more format specific (e.g .img, .tar etc). The word 'image' is synonymous with 'backup' or 'file' in this context
