What is MD5 mismatch error in Android and why
does it occur?

During backup, an md5 file is created to store information about the backup like date, naming convention etc if the file gets altered, renamed, corrupted or is incompatible,  then the information on the md5 file will differ from what the backup actually contains and also differ from what the phone expects.

How to fix md5 mismatch error

For anyone who has had the Clockworkmod
Recovery MD5 mismatch! error, these are much easier fixes:

Method 1: ADB method

1. Download and unpack MTK droid tools to your PC
2. Enable USB debugging on your phone
3. Install PdaNet on your PC with the phone connected
4. Launch MTK droid tools on your PC then click ADB
Type:
adb shell (press enter)
# cd /sdcard/clockworkmod/
backup/backup.folder.name (press enter)
# rm nandroid.md5  (press enter)
# md5sum *img > nandroid.md5  (press enter)
5. Go try flashing the Clockworkmod backup in recovery mode again

Method 2: Nandroid.md5 edit

1. Open the Clockworkmod backup folder
2. Open the Nandroid.md5 file using notepad
3. Delete all the content of the file then save
4. Go try flashing the Clockworkmod backup in recovery mode again
